Eligibility:

You may be eligible if you are a current or former participant or a beneficiary or alternate payee of a participant or former participant of the plan at any time between June 5, 2014 and October 31, 2022.

You have an active account if you have an individual investment account in the plan with a balance of more than $0. You are a former participant if you had an active account with a positive balance during the above class period but did not have an account with the plan with a balance greater than $0 as of September 30, 2022. Beneficiaries or alternate payees are beneficiaries or alternate payees of plan participants who had positive account balances during the above class period but did not have an active account in the plan as of September 30, 2022.

Typical Settlement Amount:

Payments will be determined according to a Plan of Allocation. If you are a current participant or the beneficiary or alternate payee of a participant and you have an active account in the plan, you do not need to do anything to receive your portion of the net settlement amount, which will be put into your individual account in the plan.

If you are a former participant or the beneficiary or alternate payee of a former participant, you must submit a Former Participant Claim Form to receive your share, either in the form of a check or as a tax-qualified rollover into another individual retirement account.
