Benefits:
- Credit monitoring and identity theft protection: For those who signed up for the previous offer of two years of these services, an additional year will be provided. For those who did not sign up, two years of these services will be provided.
- Reimbursement of ordinary losses: Up to $200 for reimbursement of expenses incurred in dealing with the data breach, including up to six hours of lost time at $25/hour.
- Reimbursement of extraordinary losses: Up to $8,000 for other losses arising from identity theft or financial fraud, provided certain conditions are met: (1) It is an actual, documented, unreimbursed loss that has not been compensated by another party. (2) It is attributable to this data breach. (3) It is not already covered by the ordinary loss provision above. (4) You made reasonable efforts to avoid, mitigate, or seek reimbursement elsewhere for the loss.
If all valid claims add up to more than $750,000, payments will be reduced pro rata.
