- who received one of three letters (dated August 5, 2013, August 12, 2013, and September 12, 2014) from the Hospital notifying them that they may be at increased risk of identity theft as a result of inappropriate access to their patient information, and
- whose patient information was inappropriately accessed during the period from December 19, 2009 through December 19, 2014.
Claimants can file for a reimbursement of losses and one year of credit monitoring.
