Eligibility:

You may be eligible if you are an individual to whom a notification about the data breach was sent, by or on behalf of North Shore Pain Management.

Typical Settlement Amount:

Benefits:
1. Credit monitoring: Three years of Identity Essentials credit monitoring and identity theft insurance.
2. Reimbursement of economic losses and lost time: Up to $150 per class member for documented losses, including for up to three hours of time, at $30/hour or $60 maximum.
3. Reimbursement for extraordinary economic losses: Up to $1,500 per class member for extraordinary economic losses which are related to the data breach, have not otherwise been reimbursed, supported by documentation, and meet all requirements set out in the Claim Form.
4. Alternative payment: Cash payment of up to $25, as an alternative to the above reimbursement and credit monitoring.
The reimbursement and alternative payment benefits are capped at a total of $200,000. If all claims made add up to more than this amounts, benefits will be reduced on a pro-rata basis.
