Benefits:
(1) Ordinary loss compensation: Up to $500 for expenses incurred in dealing with the data breach, including up to five hours of lost time at $500/hour and a statutory benefit of $125. Capped at a total of $150,000 for all class members. If claims exceed this amount, they will be reduced on a pro rata basis.
(2) Extraordinary loss compensation: Up to $5,000 in extraordinary unreimbursed monetary losses. Class members must show that (a) the loss is actual, documented, and unreimbursed monetary misuse or fraud loss; (b) it was more than likely caused by the data breach; (c) it occurred between July 30, 2020 and the end of the claims period; (d) it is not already covered by one or more of the categories in Question 8 of the Notice; and (e) you made a reasonable effort to avoid, or seek reimbursement for, the loss, including exhausting all available credit monitoring insurance and identity theft insurance.
(3) Two years of three-bureau credit monitoring services.
