- Are not affiliates, legal representatives, attorneys, heirs, assigns, officers, directors, or employees of the Methodist Hospitals or any entity in which the Methodist Hospitals has a controlling interest.

Benefits: (1) TransUnion Interactive’s myTrueIdentity credit monitoring and identity restoration services for two years. (2) Reimbursement of documented, unreimbursed economic losses of up to $3000 and $300 for lost time (at $20/hour) that are related to the data breach, not otherwise reimbursed by TransUnion or other parties, supported by documentation, and meet all other requirements set forth in the Reimbursement Form and Settlement Agreement.
