Typical Settlement Amount:You must fill out and submit both a claim form and a W-9. Part of the settlement will be considered wages, reported to the IRS on a Form W-2, and the rest will be considered interest and penalties, reported to the IRS on a Form 1099. Your payment will be determined based on the number of shifts you worked during the class period and the number of class members who submit valid claims.
