You may be eligible if you are an insured or a medical provider who submitted a claim for treatment to Infinity insureds, between November 15, 2012 and April 22, 2020, where charges for treatment arising from a motor vehicle accident were submitted for less than the amount allowed under the Schedule of Maximum Charges before application of the insured’s 20% co-pay (a “lesser charge”) and Infinity paid such charges at 80% of the Lesser Charge, in violation of Florida law.
You may also be eligible to join this settlement if you currently have a similar case pending against the defendant companies. Deadline to opt in: June 1, 2020.
