Eligibility:

You may be eligible if you were a patient of a physician or physician’s practice licensed under Florida statutes, and who, since March 9, 2009,

  • Made a request via a legal representative to the physician or physician’s practice for copies of your medical records,
  • For which HealthPort/CIOX billed you or your representative more than 25 cents per page after the first 25 pages, and
  • You paid these charges.

The charges may have been paid directly by you, paid to by you your legal representative, or deducted from a settlement owed to you. This case does not cover requests for medical records made to hospitals, which are governed by a different law.

Varies. The calculation will be as follows: HealthPort’s full invoice charges (excluding shipping and taxes), minus $1 per page for the first 25 pages and 25 cents for each remaining page, plus 4.78% interest per annum from the date of the invoice.
