Eligibility:

You may be eligible if you went to a hospital and received out-of-network emergency services from Emergency Physicians of Texas or another emergency provider affiliated with the defendants in this case between July 9, 2015 and January 29, 2021.

Benefits: (1) Refund of amounts you paid; (2) Write-offs of any amounts billed to you that have not been paid. Note that the Allowable Amount must have been paid by your insurance company, or you must pay it now, in order to be eligible for a refund or write-off.

Example: The amount billed for your service was $700. The Allowable Amount for your insurance company is $400. You are billed for the remaining $200. You pay $100 and still owe $200. You can have a refund for the $100 and a write-off of the $200, but only if the $400 has been paid. If your insurance company has not paid it, you must pay it now in order to receive benefits from this settlement.
