You may be eligible if you went to an in-network hospital for your insurance in Arizona and had anesthesia from Anesthesia Physicians of Arizona or another anesthesia provider affiliated with the defendants in this case, between April 15, 2017 and July 27, 2020. If you received a Notice for this settlement appeared in the records of the defendants and are likely to be eligible.
