Eligibility:You may be eligible if you received a notice about the security incident announced by DCA or one of its allied dental practices in October 2020, December 2020, or April 2021. You may also be a Settlement Subclass member if your notice said that your Social Security number, financial account, bank account or driver’s license number were potentially accessed due to the security incident.
Typical Settlement Amount:Benefits: (1) Identity and asset protection: Two years of Identity Guard identity protection, including data breach notifications, dark web monitoring, and $1 million identity theft insurance. Class members may use the activation code in the notice to enroll in the service. If you did not receive a written notice of the settlement, you must submit a claim form to receive an Identity Guard activation code. (2) Loss reimbursement for class members: Up to $2,000 for documented losses that were more than likely caused by the security incident, including up to two hours of time at $20/hour (but only if at least one hour was spent). (3) Additional loss reimbursement for subclass members: Up to another $3,000 for documented losses that were more than likely caused by the security incident, including up to another two hours of documented time spent dealing with the security incident.
Cap of $5,000 per individual. These benefits have an aggregate cap of $3 million. If the aggregate cost of these benefits (including Identity Guard services) exceeds this amount, claims will be prorated to fit within the $3 million allotted.
