If you are an Unknown Class Member, you must submit a claim form and either (a) a receipt or other document showing your purchase of a class child seat showing the date of purchase, or (2) identification of the serial number for your class child seat and the manufacturing date (which must be between August 14, 2016 and September 30, 2019).
