You may be eligible if you received a notice from Banner Health that your personal information may have been compromised in the security incident that occurred in June and July of 2016.

Typical Settlement Amount:

Three benefits: (1) Two years of credit monitoring, (2) Ordinary cash reimbursement for up to three hours of undocumented lost time or other losses up to $500, and (3) Extraordinary cash reimbursement for up to 15 additional hours of documented lost time or documented expenses of up to $10,000.
