Eligibility:

You may be an eligible EIP Member if
- You are an Eligible Injured Person under a Florida auto policy issued by one of the defendants in this case.
- You were injured in an auto accident covered by the PIP coverage of one of defendants’ Florida auto policies.
- The PIP coverage was subject to a deductible.
- Payment of the PIP benefit was calculated by applying the PIP deductible, or a portion of it, to a statutorily authorized limitation instead of to the full amount billed for your medical services.

You may be an eligible Provider Member if
- You are an assignee of an Eligible Injured Person under a Florida auto policy issued by one of the defendants in this case.
- The Eligible Injured Person was injured in an auto accident covered by the PIP coverage of one of defendants’ Florida auto policies.
- The PIP coverage for the Eligible Injured person was subject to a deductible.
- Payment of the PIP benefit was calculated by applying the PIP deductible, or a portion of it, to a statutorily authorized limitation instead of to the full amount billed for medical services.
