Two groups have been identified.
Group 1 are class members whose contact information was identified through A Place for Mom’s records. These class members should have received a shorter version of the Notice through the US mail or by e-mail identifying them as members of Group 1. They do not need to submit a claim form.
Group 2 are potential class members whose contact information was not available through the company’s records. These class members must submit a claim form.Typical Settlement Amount:
