You must attach to your claim form either (1) the original or a copy of your receipt showing the expiration date and/or more than the last five digits of your personal card number from the transaction with Centerplate at the convention center on September 5, 2015, or (2) an original or a copy of your personal credit or debit card statement showing your transaction with Centerplate at the convention center on September 5, 2015.

Eligibility:

You may be eligible if you attended the 2015 National Book Festival and used your personal debit or credit card to make a purchase at the Walter E. Washington Convention center in Washington DC on September 5, 2015 and Centerplate gave you an electronically printed receipt at the point of sale on which the expiration date and/or more than the last five digits of your credit or debit card number were printed.

Typical Settlement Amount:

Benefit: Payment of $1,000. The Settlement Administrator will provide you with a form you can use to prevent withholdings from being subtracted if you can certify that you are not subject to withholding; if you do not submit that form with your certification, mandatory backup withholding will be subtracted from your payment.
